For your protection, all address changes must be submitted in writing with a physical signature from the owner or the owner’s representative. Electronic signatures will not be accepted. Please download and fill out the Apache Owner Address Change Form and email it to [email protected].

Or they can be faxed to 713-296-6482
For an individual name change, provide legal documentation that proves the name change request. Acceptable documents include marriage certificates, divorce decrees or court-approved documents. A driver’s license or passport is not acceptable.
For a company name change, please provide a copy of the Certificate of Merger, including tax ID or the Certificate of Name Change, including tax ID.
Many factors affect your payment amount, including market conditions, fluctuating commodity prices, regulatory or contractual changes, production volumes, weather conditions and well downtime.
If Apache does not have a valid tax identification or Social Security number for an owner, the Internal Revenue Service requires us to withhold a percentage of the revenues owed. This is referred to as federal withholdings and will appear on your Revenue Statement as “NRA/Back-up Withholding.” To update your information, please download and complete a W-9 form from the IRS website and return it to [email protected].
